All Clients First Visit - Please arrive 10 min early for your first visit in order to process required paperwork.
Returning Clients - Please arrive 5 min early for in-processing time.
*Massage Client Intake Forms - Please print a copy of the "Client Intake Form" and bring with you to your first visit.
*Massage Client HIPAA Agreement Form - Please print and bring a completed copy of your signed "HIPAA Agreement" with you on your first visit.

Cancellation Policy - Our time together is important. Unless there is an emergency, it is requested that you cancel your appointment 24 hours in advance or pay the missed appointment fee of $10.
Missed Appointment Fee - If you accidentally forget your appointment, the fee is $10 and will be applied to your next visit.
Missed Gift Certificate Appointment - Our time is valuable, if you forget and miss your appointment when using a pre-paid gift certificate, you will forfeit the use of the certificate.
Lateness Policy - We request that you arrive early for your appointment. In order for us to uphold our professional standards of being on time. Massage clients arriving late will receive the remainder of their scheduled massage time and will be charged the full amount for the original service scheduled. All Clients arriving more than 10 minutes late may be rescheduled or your time will be shortened to accommodate our schedules.
Promotions - Promotional discount offers will be calculated when services are rendered (unless purchased online).
Discounts - Discount coupons, cards, codes are not valid with other offers and will be calculated when services are rendered.
Sickness Policy - Rocky Mountain Massage recognizes that both massage therapists and clients are vulnerable to infections and therefore ask clients to cancel appointments when they are feeling unwell.
If a client has any of the following contagious illnesses the massage session will be rescheduled:
Diarrhea, Vomiting, Fever, Chicken Pox, Measles, Mumps, Meningitis, Hepatitis A, Conjunctivitis, Rubella, Head Lice, Impetigo, Influenza, Meningitis, Polio, Ringworm of the body, feet or scalp, Scabies, Thrush, Whooping Cough and the Common Cold.
